What to have ready

  1. Your visit reason — primary care, functional medicine, women’s health, weight loss, aesthetics, or another concern.
  2. Preferred days or times, plus whether you are a new or returning patient.
  3. Insurance information if you plan to use coverage (or let us know if you are self-pay).

After you are scheduled, complete new patient paperwork before your first visit and use the patient portal when prompted.
